Patient App Set Up and Registration

Step 1: Download the app from the app store. Once complete, launch the app. 

Step 2: To register for your account, fill out the information on the registration screen. All fields are mandatory to fill before completing your registration. Once complete, click Confirm.

Step 3: On the next page, you will enter your mobile phone number and read and confirm by checking the box that you have read and agreed to the Privacy Policy and Platform User Agreement & Terms of Use. This must be checked to proceed. Once you click Proceed, a verification code will be sent by text message to your telephone number to confirm your information.

Step 4: Once you have received the verification code, enter it in the field provided. If you do not receive the code, click on resend verification code to resend the code. Once your code has been entered, click Continue to finalize your registration.
